4+1 MLIS Program Application Form

BSIS students at the end of their sophomore fall semester should complete this form to apply to the MLIS 4+1 Program.

Note: Interested students would be required to complete all BSIS benchmark courses (MATH115 or higher, PSYC100, INST126, INST201, and STAT100) and one BSIS core course (INST311 Information Organization) by the end of their sophomore fall. MATH115, INST126, and STAT100 must be completed with a grade of C- or better. INST152, INST 201, INST311 and PSYC100 must be completed with a grade of B or better. Eligible students will apply after the conclusion of their sophomore fall, receive admissions offers in their sophomore spring, and begin classes in their junior fall. Students who are finishing up required entrance courses may be conditionally admitted pending successful completion of fall semester coursework.
